Pennsylvania splits the bill in two. PennDOT sets the annual inspection sticker at $12.00, the same in every garage in the state. What the station charges to actually perform the inspection is not set by PennDOT — their own words are that inspection fees are posted at official inspection stations. The repairs are not in the price
PennDOT states it directly: these fees do not include repairs to correct vehicle defects. An inspection is a diagnosis, not a remedy. If the car fails, what you have bought is the finding — and the quote to fix it is a separate conversation you are entitled to have with a different garage.
That matters most when you are buying. A seller saying “it needs a bit of work for inspection” is describing an unpriced liability, and the inspection fee tells you nothing about its size.
Emissions in Pennsylvania is a regional question
Pennsylvania does not run one statewide emissions programme. PennDOT describes it as customised “to meet specific regional air-quality issues and improvement goals”, and directs drivers to information for their own region rather than publishing a single county list the way North Carolina does. Whether you need an emissions test, and what form it takes, depends on where the vehicle is registered.

The fixed half is public. The variable half is posted on a wall — which means it can be compared, and most people never do.

What to ask before you hand over the keys
Because half the bill is set locally, a two-minute phone call is worth more in Pennsylvania than in a capped state. Three questions get you the whole number.
“What do you charge for the inspection itself?” This is the part PennDOT does not set. It has to be posted at the station, so the answer exists and is not a negotiation — you are asking them to read you a number off their own wall.
“Does that include the sticker?”The $12.00 annual sticker is a pass-through, and some quotes fold it in while others state it separately. Two garages quoting “forty dollars” can mean different totals depending on which side of that line they are speaking from.
“What happens if it fails?” PennDOT is explicit that the fee excludes repairs. Ask whether the inspection charge still applies on a failure, and whether they will re-inspect after you have had the work done elsewhere. Neither answer is fixed by the state.
Buying privately in Pennsylvania
PennDOT's own framing of why the programme exists is narrow and worth reading literally: Pennsylvania requires vehicle safety inspections to ensure that vehicles are maintained for safe operation. That is a statement about maintenance, not about history — and it is the gap between what a sticker proves and what a buyer assumes it proves.
A fresh sticker on a car you are considering means a licensed station found it roadworthy on the day it looked. It does not mean the car has never been in a collision, that the odometer reading is genuine, that it was not written off and rebuilt in another state, or that a lender has released its claim. Those questions are answered by the record attached to the VIN, and they are the ones that change what the car is worth.

It does not tell you the car was written off in another state, that the odometer was wound back, or that a lender still has a claim on it. Read the VIN before you agree a price.
How Pennsylvania compares
There are three ways a state can handle this, and Pennsylvania is the middle one. Knowing which model you are in tells you whether shopping around is worth the phone calls.
Pennsylvania — part fixed

North Carolina — capped
